J ob Summary

The Outreach Engagement/Medi-Cal Navigator is a distinct position trained to offer information and enrollment assistance with programs available to low and moderate income residents of the San Gabriel Valley, such as Medi-Cal, Covered California, Medi-Cal Access Program and county-based subsidized healthcare programs and services. Coordinates strategies with the Coordinated Entry System (CES) to prioritize and coordinate with homeless individuals for assistance with health insurance. The position functions as both an educator about the health insurance programs and an ombudsman to the consumer to enroll or navigate the system to troubleshoot for an existing application. Conducts follow up calls to confirm enrollment, utilization of health services and annual redetermination.

Conducts community outreach presentations and attends community events. This position is part of a dynamic outreach and education team.

Job Requirements

M inimum Education Requirement :

High school diploma or equivalent required. Bilingual in Spanish. Ability to work with computer systems.

Minimum Experience Requirement :

One (1) year experience with screening, enrollment, retention and trouble-shooting of health care benefits programs for low-income communities preferred. Ability to establish trusting, supportive relationships in an ethnically diverse population with empathy and compassion. Excellent customer service skills required. Responsible and able to work independently. Confidentiality requirements and good communication skills.

Minimum License Requirement :

Completion of the Maternal Child Health Access “We’ve Got You Covered” training within 45 days of hire/transfer (based on training availability). CAC Certification (Certified Application Counselor) required or completion of Covered California training (based on training availability). Must have own car, valid CA driver’s license and proof of insurance required.

Pay Range:

$21.00 - $31.50
